About the Role

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. As a Case Manager, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Conduct thorough patient assessments and develop comprehensive care plans in collaboration with the care team.
  • Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
  • Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
  • Supervise LPNs and HHAs, ensuring they provide high-quality care and adhere to our standards.
  • Complete documentation timely and accurately, ensuring seamless communication with the care team.
  • Regularly communicate patient progress to the clinical manager and care team, ensuring everyone is informed and aligned.
  • Plan and provide staff education, enhancing the skills and knowledge of our team members.
  • Perform on-call responsibilities and on-call services to patients/families as assigned, demonstrating our commitment to 24/7 care.
  • Participate in clinical development and continuing education programs, staying up-to-date with the latest best practices and research.
Qualifications
  • One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
