Job Opportunity

Medi Home Hospice, a leading provider of home healthcare services, is seeking an experienced Part-Time PRN Hospice Social Worker / MSW to join our team in Westmoreland, Allegheny, and Butler Counties, PA.

As a Hospice Social Worker, you will provide social work services in accordance with the physician's plan of treatment, working closely with the interdisciplinary team to assess and support patients and families.

Key Responsibilities:

  • Provide assessment and support to patients and families
  • Work with the interdisciplinary team to develop and implement care plans
  • Provide bereavement counseling and support
  • Serve as a liaison with community resources

Requirements:

  • Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education
  • Currently licensed to practice as a social worker in the state of PA (preferred)
  • Minimum of one-year Hospice experience preferred
  • Valid Driver's License
