About the Role:

We are looking for a Physical Therapist to provide high-quality care to our clients in their homes. As a Physical Therapist, you will work as part of a multidisciplinary team to develop and implement individualized care plans, conduct assessments, and provide education to clients and their families.

Responsibilities:
  • Conduct diagnostic tests and measurements to assess client needs
  • Develop and implement individualized care plans to promote client independence and safety
  • Provide education and instruction to clients and their families on rehabilitative care and activities
  • Accurately document client care management and services provided
Qualifications:
  • Current Pennsylvania Physical Therapist license
  • Minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
