Job Summary:

As a Licensed Practical Nurse with HexaQuEST Health, Inc., you will play a vital role in providing high-quality patient care in our community health settings. This position requires a strong commitment to delivering compassionate and individualized care to our patients.

Key Responsibilities:

Conduct thorough assessments of patients to identify their health needs and develop personalized care plans.
Administer medications and treatments as prescribed by physicians and other healthcare professionals.
Collaborate with healthcare teams to ensure seamless care coordination and communication.

Requirements:

Current licensure as a Licensed Practical Nurse in the state of practice.
Ability to work in a fast-paced community health setting with minimal supervision.
Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.

COVID-19 Precautions:

COVID testing is not required for vaccinated associates unless symptomatic or if there is an exposure/outbreak.
