We are seeking a compassionate and skilled Hospice Clinical Social Worker to join our team at AccentCare, Inc. As a Hospice Clinical Social Worker, you will play a vital role in providing emotional, social, and spiritual support to patients and their families.
Key Responsibilities:
- Assess patients' and families' needs and develop individualized care plans to address their emotional, social, and spiritual needs.
- Provide counseling and support to patients and families to help them cope with the end-of-life process.
- Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services.
- Document patient progress and maintain accurate records.

Requirements:
- Master's degree in Social Work (MSW) or related field.
