Job Summary

We are seeking a skilled Home Health Intake Licensed Practical Nurse (LPN) to join our team at Interim HealthCare. As a Home Health Intake LPN, you will be responsible for providing exceptional patient care while enjoying the flexibility and autonomy that comes with this role.

Key Responsibilities
  • Act as the primary point of contact for referral sources, outside agencies, health professionals, and field staff.
  • Coordinate patient care by receiving and reviewing orders from referral sources, and entering information into our management information system.
  • Communicate effectively with patients, families, and referral sources to ensure seamless care transitions.
  • Collect and enter customer information into our management information system to ensure accurate and up-to-date records.
  • Work collaboratively with clinical staff to adhere to standards of practice for nursing and applicable law and regulations.
Requirements
  • Graduate of an accredited practical nursing program and active LPN license in Colorado.
  • Minimum of one (1) year of LPN experience, preferably in home healthcare.
  • CPR certification and QAPI experience preferred but not required.
  • Able to perform home visits on a PRN basis and pass a background check and drug test.
  • Knowledge of state and federal home health regulations and clinical proficiency in nursing, strong communication skills, and compassion.
