We are seeking a dedicated and compassionate Admission Nurse to join our Hospice team. This is a full-time position with excellent benefits.

Responsibilities
  • Conduct ongoing assessments of patients' physical status and psycho-social well-being.
  • Develop individualized care plans in collaboration with the Interdisciplinary Group (IDG), patients, families, and attending physicians.
  • Provide education and interventions to manage patients' pain and symptoms, assessing their response to treatment.
  • Evaluate equipment, supplies, medications, and services required for patient care, ensuring timely revisions to the IDG plan as needed.
  • Maintain accurate and timely documentation of patient services provided.
Requirements
  • Active, unrestricted Registered Nurse license in Michigan.
  • At least one year of relevant experience as a Registered Nurse.
  • Preferred qualifications include one year of management experience, one year of hospice experience, CPR certification, community health experience, and a wide range of Medical-Surgical experience.
