Main Responsibilities:

  • To provide one-on-one physical therapy sessions to patients in their homes, focusing on mobility and functional improvement.
  • To create and implement customized treatment plans that align with patients' goals and preferences.
  • To educate patients and caregivers on proper exercises and techniques to promote optimal outcomes.
  • To maintain accurate records and communicate effectively with the interdisciplinary team.

Essential Qualifications:

  • A Master's degree or higher in Physical Therapy from an accredited institution.
  • A valid license to practice in the state of TX and current certification in PT liability insurance.
  • A minimum of 3 years of experience in Home Health or Rehab settings.
  • The ability to establish strong relationships with patients, families, and colleagues.
