Job Title: IDD Qualified ProfessionalAbout the Role:

We are seeking a highly skilled and compassionate IDD Qualified Professional to join our team at RHA Health Services. As a key member of our interdisciplinary team, you will play a vital role in providing person-centered services to individuals with intellectual and developmental disabilities.

Key Responsibilities:
  • Coordinate IDT meetings and develop individualized service plans (ISPs) that meet the unique needs of each individual.
  • Assist individuals in identifying and communicating their requests and needs for services and supports through direct interviews.
  • Supervise, integrate, and coordinate person-centered plans, monitoring progress towards personal, clinical, and functional outcomes.
  • Initiate periodic reviews, investigations, modifications, and adjustments by soliciting feedback from individuals and their families.
  • Review assessments completed by the interdisciplinary team to ensure that individuals receive the best possible support.
  • Supervise and write person-centered plans based on assessments, interviews, and observations.
  • Oversee residential activities, including home appearance, leisure materials, cleanliness, and community integration, to ensure a safe and healthy environment.
  • Visit vocational centers, residential homes, and other service areas to assess the implementation of person-centered plans.
  • Report to the Administrator and collaborate with colleagues to maintain communication, provide feedback, and standardize procedures.
  • Monitor activities to ensure quality assurance and state survey improvements.
  • Serve as a link between individuals, their families, and legal guardians to gain approval for services provided.
Requirements:
  • Bachelor's degree in Social Work, Social Services, Human Services, or Special Education required; Master's degree a plus.
  • Minimum 2 years of experience working with individuals with intellectual and/or developmental disabilities required; 1 year of experience if paired with a Master's degree.
  • Working knowledge of applicable regulations that apply to our service offerings.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Supervisory/management experience and experience writing person-centered plans preferred.
  • Must be at least 18 years of age.
