TransForce is seeking a dedicated and compassionate Contingent Category II Community Outreach Worker I/Community Outreach Worker II to join our mission-driven team. The selected individual will facilitate client enrollment into these services and provide ongoing case management support. This position will report directly to the Community Program Director.

Key Responsibilities
(Community Outreach Worker I)
- Identify and recruit potential clients within the community through interviews and screenings.
- Provide information to individuals and families regarding program objectives, eligibility, and services.
- Conduct assessments of clients' living conditions to ensure a safe and healthy environment.
- Act as a liaison between clients and community resources, including various social services agencies.
- Schedule and remind clients of appointments with healthcare providers, providing support as needed.
- Assist in client retention through follow-ups and outreach efforts.
- Facilitate clinic processes by ensuring clients are registered for services.
- Help organize community events such as health fairs and workshops.
- Participate in staff meetings, reporting on outreach activities and community concerns.
- Maintain accurate records of outreach efforts and update databases accordingly.
- Foster positive relationships with community members.
- Provide crisis intervention and individual counseling, along with basic health education.
- Conduct initial assessments to determine client needs.
- Engage with neighborhood groups and attend community meetings to promote services and address community issues.
- Evaluate educational materials and methods to meet client community needs.
- May provide guidance and training to other outreach workers.
- Perform all duties of a Community Outreach Worker I.
(Community Outreach Worker I)
Education: High school diploma or GED.
Experience: None required.
Other: Additional related experience or education may substitute for the requirements on a year-for-year basis.
(Community Outreach Worker II)
Education: High school diploma or GED.
Experience: Two years in community outreach, healthcare, research, or social services.
Other: Additional related experience or education may substitute for the requirements on a year-for-year basis.
Required Skills and Abilities
Ability to communicate effectively, establish relationships, operate office equipment, write reports, and maintain confidentiality. Candidates should be able to work in various weather conditions and manage sensitive situations with discretion.
Conditions of Employment
Employees may be required to use personal vehicles for official duties and possess a valid driver's license. Candidates may undergo medical inquiries and background checks as part of the hiring process. This position is not eligible for participation in the Bargaining Unit (Union).
Salary Range
Community Outreach Worker I: $34,613 - $38,500, commensurate with education and experience.
Community Outreach Worker II: $38,700 - $44,000, commensurate with education and experience.
