Job Overview

The Advanced Practice Professional (APP) plays a vital role in the comprehensive management of digestive disease patients. This includes biological, social, nutritional, and psychological assessments.

The APP coordinates involvement of medical specialty teams to develop a plan for each patient, reviews the plan with the clinical team, participates in directing care, performs diagnostic/therapeutic procedures, and coordinates discharge planning and outpatient management.

The APP also participates in the education of residents and medical/nursing students.

In this role, you will provide healthcare services consistent with state law and professional licensing requirements. Responsibilities include managing care for digestive disease patients, completing daily rounds, performing integrated assessments, and documenting findings in the medical record.

You will also facilitate patient discharge, develop individualized plans of care, and participate in inter-disciplinary approaches to patient care and program development.

This is an opportunity to utilize your skills in caring for patients across the age continuum, identifying complex situations, and providing emergency care when needed.
