About the Role

We are seeking a skilled Physical Therapist to join our team at CommuniCare Family of Companies. As a Physical Therapist, you will play a vital role in providing high-quality care to our residents and patients.

Responsibilities
  • Evaluate and develop treatment plans for patients/residents, ensuring a comprehensive and individualized approach to care.
  • Implement treatment plans through direct treatment and education of the care team, including providing necessary supervision for Physical Therapist Assistants.
  • Participate in interdisciplinary meetings to share clinical findings and make recommendations to the rehab team, as well as conducting staff and family training and education when necessary.
  • Opportunity to work at multiple centers, allowing you to serve a wide variety of patient needs and contribute to the growth and development of our organization.
Requirements
  • A Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited program.
  • Hold a valid or pending State Physical Therapy License.
