Job Title: Centralized Follow Up NurseAbout the Role

We are seeking a skilled and compassionate Centralized Follow Up Nurse to join our team at Planned Parenthood. In this critical role, you will work closely with the Centralized Follow Up and Patient Services Departments to develop and implement centralized follow-up management strategies.

Key Responsibilities
  1. Develop and manage lab result management processes in accordance with PPHP Medical Standards and Guidelines (MSG) and Patient Services Procedures.
  2. Manage lab results, patient notification, and follow-up as per physician-approved plans, adhering to PPHP Medical Standards and Guidelines (MSG) and Patient Services Procedures.
  3. Support the management of the pre-natal program, ensuring patients receive coordinated care.
  4. Provide pre-natal education to patients, promoting their health and well-being.
  5. Track and follow up on labs and referrals, missed appointments, delivery outcomes, and postpartum visits.
  6. Maintain accurate and up-to-date pre-natal logbooks and perform quarterly pre-natal audits as required.
  7. Handle patient calls, routing them to the appropriate department for further assistance when necessary.
  8. Offer comprehensive telephonic nursing assessments, evaluations, education, and advice to patients within affiliate protocols.
  9. Perform post-surgical abortion phone calls to patients within 24-48 hours of the procedure, completing follow-up paperwork.
  10. Document all communication with patients accurately, efficiently, and in accordance with PPHP standards.
  11. Maintain electronic logs and notify pharmacies of new prescriptions and refills as ordered by the provider.
  12. Collaborate with PPHP contracted laboratories as needed.
  13. Complete audits accurately and within specified timeframes, as instructed by the Director, Quality & Clinical Integration.
  14. Provide exceptional telephone customer service to internal and external customers.
  15. Effectively communicate requests and respond using appropriate procedures for contacting clinics and/or clinic staff.
  16. Maintain a thorough understanding of PPHP's organization to answer questions and direct calls correctly.
  17. Complete quality assurance monitoring tools as specified for the department.
  18. Provide nursing coverage at surgical sites as needed and directed.
  19. Assist in all aspects of abortion care, including acting as Charge Nurse for patient flow.
  20. Provide counseling for patients undergoing surgical abortion and oversee patient care during procedures.
  21. Supervise medical assistants in the post-procedure room.
  22. Create a compassionate environment by offering emotional and psychological support to patients and their families.
  23. Ensure quality care by adhering to PPHP standards, measuring health outcomes, and making necessary adjustments.
  24. Protect patients and staff by adhering to infection-control policies and regulations.
  25. Document patient care services accurately and maintain a cooperative relationship with the healthcare team.
Requirements
  • Registered Nurse with a current NYS license in good standing.
  • Current CPR/BLS and ACLS certification.
  • One-year experience in Women's Healthcare, abortion services, or recovery room setting preferred.
  • Bilingual English/Spanish skills a plus.
  • Ability to use a computer for charting, documenting, and communicating.
  • Excellent verbal, written, and interpersonal communication skills.
  • Ability to work autonomously with a set of protocols.
  • Comfortable working in a paperless environment with electronic patient information management.
  • Willingness to work flexible hours, including evenings and weekends.
  • Ability to maintain strict confidentiality and adhere to HIPAA guidelines.
  • Familiarity with electronic health records and practice management systems.
