Key Responsibilities
- Serve as an advocate for patients and their families throughout the entire care journey.
- Monitor patient progress, intervening as necessary to ensure optimal recovery.
- Oversee all facets of discharge planning, ensuring a safe and effective transition from care.
Qualifications
The Case Manager RN employs advanced nursing expertise to coordinate comprehensive care for a designated patient demographic, facilitating their journey through the healthcare continuum.
Required Qualifications:
- Bachelor’s degree in Nursing
- Minimum of three (3) years of experience in case management or discharge planning
- Certification in Case Management (CCM) is preferred
- Active Registered Nurse license in New Jersey or Pennsylvania
