We are seeking a skilled Hospitalist Nurse to join our team at UnityPoint Health. As a key member of our hospital care team, you will play a vital role in coordinating and managing patient care from admission to discharge.
Responsibilities- Collaborate with the hospital care team to ensure timely completion of lab results, consults, and orders.
- Participate in morning report and census coordination to ensure seamless patient care.
- Actively participate in quality improvement initiatives and review core measure indicators to ensure team goals are met.
- Assess patient needs and collaborate with families to set up follow-up care upon discharge.
- Obtain medical records and assist with medication reconciliation.
- Coordinate care with patients and families, including setting up care team meetings and providing post-discharge homecoming calls.
- Fill out documents for patients and families regarding hospital stay and follow up on lab and pending items post-discharge.
- Graduate of an accredited program for Registered Nurses.
- 3-5 years of nursing experience preferred.
- Current license to practice nursing in the State of Iowa/Illinois for Registered Nurse. BLS certification.
