Key Responsibilities
- Manages incoming referrals and assigns appropriate clinicians or therapists for prompt patient assessments, ensuring authorization and scheduling of start of care (SOC) visits.
- Oversees the evaluation of patient home health benefits, medical necessity, and continuous insurance approvals.
- Continuously assesses patient needs, ensuring that care is tailored to individual requirements, meets eligibility criteria, and aligns with physician directives.
- Reviews daily assessments and care plans, collaborating with clinicians to provide recommendations as necessary.
- Coordinates comprehensive care across various disciplines, including physicians, medical equipment suppliers, family caregivers, and other healthcare professionals.
- Monitors lab results and clinical diagnostic tests, ensuring timely physician notifications and necessary adjustments to care plans.
- Performs additional duties as assigned.
Education & Experience
- Active RN licensure in the state of Texas
- Current CPR certification
- Valid Driver's License, vehicle insurance, and access to reliable transportation
- Minimum of one year of professional nursing experience
