Job Description

We are seeking a dedicated Registered Nurse to promote wellness, improve outcomes, and self-care within a defined patient population. The RN will provide support, education, and assistance in the prevention and/or maintenance of disease and/or health and wellness state.

Key Responsibilities:
  • Collaborate with patients, families, and providers to develop and implement individual health plans.
  • Provide support and education to patients with chronic conditions, ensuring they have the knowledge and resources needed to manage their health effectively.
  • Coordinate care and follow-up for patients who receive care in outpatient facilities, ensuring seamless transitions and optimal outcomes.
  • Assist with prescription refill requests and patient education coordination, streamlining the healthcare experience for our patients.
Requirements

To be successful in this role, you will need:

  • Bachelor's degree in Nursing, Associate's degree in Nursing, Diploma RN, or Master's degree in Nursing.
  • A minimum of 1 year of Registered Nursing experience, with a proven track record of delivering exceptional patient care.
  • A valid Registered Nursing License, demonstrating your commitment to ongoing professional development.
  • Basic computer skills, enabling you to navigate our electronic health records system with ease.
  • Excellent communication skills, allowing you to build strong relationships with patients, families, and healthcare teams.
