Job Title: Health Advocate

About the Role:

Finger Lakes Community Health is seeking a dedicated Health Advocate to join our team. As a Health Advocate, you will play a critical role in educating patients on the importance of family planning services and promoting reproductive health outcomes related to cancer prevention.

Key Responsibilities:

  • Develop and implement comprehensive patient plans to ensure patients receive appropriate overall medical care.
  • Assist patients in accessing mammograms, pap smear screenings, and HPV vaccinations.
  • Provide education and outreach to patients on the importance of regular cancer screenings and preventative measures.
  • Collaborate with the interdisciplinary team to ensure excellence in patient care.
  • Manage patient roster for cancer screening around breast and cervical cancer.
  • Represent Finger Lakes Community Health at community agency/network meetings.

Requirements:

  • 1-3 years experience in counseling, education, youth work, or social work.
  • Possesses valid New York State drivers license in compliance with FLCH drivers policy.
  • Current New York State automobile insurance in compliance with FLCH underwriting agreement.
  • Excellent interpersonal, communications, public speaking, and presentation skills.
  • Intermediate computer skills.
