Willowbrook Hospice is seeking a dedicated **Licensed Practical Nurse (LPN)** to provide essential nursing care to patients in a hospice setting. This role involves delivering compassionate support to individuals facing terminal illnesses, in alignment with physician directives and the Interdisciplinary Group (IDG) care plan.
Key ResponsibilitiesThe LPN will collaborate closely with the Nurse Case Manager, Director of Nursing (DON), and the Interdisciplinary Team to:
- Deliver direct clinical nursing care, engaging with patients, families, physicians, and other healthcare professionals involved in patient care.
- Coordinate comprehensive pain and symptom management strategies, integrating both pharmacological and non-pharmacological approaches to enhance patient quality of life.
- Facilitate patient autonomy, ensuring dignity and comfort for those navigating end-of-life challenges.
- Provide educational resources and emotional support to families, grounded in the hospice philosophy of holistic care, addressing physical, psychological, spiritual, and financial needs.
- Ensure all services align with the patient’s Plan of Care (POC), actively involving the patient, family, primary care provider, and the Interdisciplinary Team.
- Communicate effectively with the Nurse Case Manager to coordinate all aspects of hospice care, including referrals for additional services such as home health aides, social workers, and chaplains.
- Educate patients and families about disease processes and expectations regarding progression.
- Participate in Continuous Quality Improvement (CQI) initiatives.
- Uphold teamwork principles and adhere to the Hospice Code of Ethics.
- Manage patient symptoms with appropriate interventions and utilize the medication formulary as needed.
- Report any changes in patient condition to the DON, particularly those necessitating adjustments in care level.
Willowbrook Hospice is committed to providing necessary orientation and training to support the LPN in fulfilling these responsibilities. Training may be conducted in-house or through external seminars, with documentation maintained in the personnel file.
Accountability StructureThe LPN will report directly to the Nurse Case Manager and the Director of Nursing, ensuring effective communication and coordination of care.
