We are seeking a skilled Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team of dedicated healthcare professionals. Responsibilities- Provide individualized care to clients, following their personalized care plans and maintaining their dignity and independence.
- Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
- Collaborate with other healthcare professionals, such as physical therapists, occupational therapists, and speech therapists, to ensure comprehensive care.
- Implement infection control policies and procedures, and report any possible communicable diseases or infections.
- Accurately document all nursing actions, communicate with family members, case managers, and physicians, and document progress and outcomes for established goals.
- Order supplies, medications, and feedings as needed to ensure adequate stock levels.
- Ensure all patient needs are met in accordance with physician instructions.
- Monitor incidents involving clients, complete incident reports, and inform case managers.
- Maintain and report faulty medical equipment to the Durable Medical Equipment department.
- Current license as an LPN or LVN in good standing from the state(s) of practice.
- Current Cardiopulmonary Resuscitation (CPR) certification.
- Ability to work in a home-like setting.
- Physical ability to bend, stoop, push, pull, sit, and walk for periods of time.
- Able to utilize proper body mechanics while lifting 50 pounds.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Must have completed post-secondary coursework to facilitate licensure as an LPN or LVN.
