Primary Care Nurse Practitioner

We are seeking a highly skilled and compassionate Primary Care Nurse Practitioner to join our team at The MedElite Group. As a Primary Care Provider, you will play a vital role in delivering high-quality, patient-centered care to residents in skilled nursing and long-term care facilities.
Key Responsibilities- Provide Comprehensive Healthcare Services: Coordinate patient care, assess, diagnose, and treat residents' physical and mental well-being.
- Collaborate with Healthcare Professionals: Work closely with a team of healthcare professionals to ensure seamless coordination of care.
- Conduct Patient Assessments: Conduct thorough patient assessments, including medical history reviews and physical examinations.
- Order and Interpret Diagnostic Tests: Order and interpret diagnostic tests to inform treatment plans.
- Develop Accurate Diagnoses and Treatment Plans: Develop accurate diagnoses and treatment plans based on patient assessments and test results.
- Prescribe Medications and Administer Treatments: Prescribe medications and administer treatments as needed.
- Provide Counseling and Education: Provide counseling and education to residents and their families on health-related topics.
- Manage Chronic Conditions: Manage chronic conditions and monitor progress.
- Commitment to Person-Centered Care: Create a safe and nurturing environment for elderly residents in the nursing home.
- Nurse Practitioner or Physician Assistant License: Current licensure as a Nurse Practitioner or Physician Assistant.
- DEA License: DEA license to prescribe controlled substances.
- Experience in Long-Term Care: 6 months of experience in long-term care settings.
- Competitive Salary: $150,000 - $200,000 per year.
