Nurse Home Visitor - Maternal Child Health

About the Role:

The Nurse Home Visitor is a pivotal member of our team, responsible for providing comprehensive nursing services to women and their families eligible for the Nurse-Family Partnership (NFP) Program. This role requires a compassionate and skilled nurse who can work collaboratively with families to promote healthy outcomes and empower them to reach their full potential.

Key Responsibilities:

  • Provide health checks and share resources with first-time expectant parents during pregnancy and until the child is two years old.
  • Travel to and visit families in their homes or community locations, building strong relationships and trust.
  • Work with families to uncover their strengths and skills as parents, promoting a supportive and nurturing environment.
  • Manage a caseload of 25-30 clients, providing individualized care and support.
  • Participate in extensive model training and ongoing reflective supervision to ensure fidelity to the NFP model.
  • Travel required for visiting families, attending training and meetings, and adhering to company and program guidelines.

Requirements:

  • Eligible candidates must be a registered nurse with a minimum of a Bachelor's degree in Nursing (BSN) and a current RN license in good standing.
  • Experience in public health, community, and/or maternal-child health is required.
  • Current CPR certification is necessary.
  • Home visiting experience is preferred, and bilingual skills (English/Spanish) are desirable.
  • A valid driver's license, meeting the requirements of the agency's insurance carrier, is required.
