Job Summary:

We are looking for a dedicated and experienced LPN/LVN to join our team. The successful candidate will be responsible for providing skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This is a rewarding opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and advance your career.

Responsibilities:
  • Provide one-to-one patient care in a home environment
  • Execute physician-prescribed plans of care and document care provided in a timely manner
  • Administer prescribed medication, treatments, and therapies
  • Conduct patient assessments and coordinate care
  • Provide health education and training to patients and their families
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
  • Six months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
