Job Summary:We are looking for a dedicated and experienced LPN/LVN to join our team. The successful candidate will be responsible for providing skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This is a rewarding opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and advance your career.
Responsibilities:- Provide one-to-one patient care in a home environment
- Execute physician-prescribed plans of care and document care provided in a timely manner
- Administer prescribed medication, treatments, and therapies
- Conduct patient assessments and coordinate care
- Provide health education and training to patients and their families
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
- Six months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
