Job Description- Evaluate resident needs through ongoing assessment and revise care plans based on changes in condition.
- Provide clinical supervision to LPNs and CNAs.
- Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate resident care plans.
- Instruct individuals and families on topics such as health education, disease prevention, and develop health improvement programs.
- Interpret and evaluate diagnostic tests to identify and assess resident conditions; modify treatment plans as indicated by residents' responses and conditions.
