Responsibilities
  • Model the WellBe values and mission in care delivery, ensuring exceptional patient outcomes and experiences.
  • Partner with patients' primary care physicians to provide comprehensive care and support.
  • Conduct initial patient screenings, physical examinations, and create assessment/plan documentation in a timely manner.
  • Order, interpret, and analyze diagnostic tests, determining the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments as appropriate, adhering to state supervisory agreements.
  • Update patient records and ensure accuracy at each appointment.
  • Deliver high-quality care while maintaining company metrics and patient needs.
  • Collaborate with the interdisciplinary team and participate in regular team meetings.
  • Maintain required licensure and certification, adhering to a professional code of conduct.
Requirements
  • Master's Degree in Nurse Practitioner studies from an accredited program.
  • Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the posted state.
  • Current board certification, either AANC or AANP.
  • BLS certification and active NPI# as an NP.
  • Active, unencumbered DEA licensure or eligibility to apply upon hire.
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics preferred.
Work Environment

This role requires driving to patient locations, including homes, hospitals, and skilled nursing facilities. The ability to lift up to 30 pounds and stand for extended periods is necessary. Fine motor skills and visual acuity are also required.
