Key Responsibilities:

  • Deliver in-home care to patients recovering from illnesses, injuries, or managing chronic conditions, often post-hospital discharge, under the guidance of an RN
  • Collaborate with a multidisciplinary team including physicians, RNs, CNAs, and aides to ensure high-quality patient care
  • Monitor patient vitals, conduct assessments, document progress, and communicate changes to the supervising RN
  • Assist the RN in implementing the patient's care plan and instructing patients on self-care techniques
  • Educate family members regarding the patient's care plan, medication management, and home safety practices
  • Work with the interdisciplinary team to coordinate patient care and facilitate discharge planning

Essential Qualifications:

  • Graduation from an accredited nursing program with an active RN license
  • Current CPR certification
  • Familiarity with state and federal home health regulations
  • Strong clinical skills, effective communication abilities, and a compassionate approach to patient care
