About the Job
  • Provide skilled nursing care to clients in their homes, promoting independence and dignity.
  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare teams.
  • Administer medications, perform wound care, and provide treatments as prescribed by physicians.
  • Document client progress, interventions, and evaluations using electronic medical records.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Graduation from an accredited nursing program and current RN licensure in New Jersey.
  • Minimum one year of recent, verifiable clinical experience, preferably in home health care.
  • Strong computer skills and experience with electronic medical records (EMR) preferred.
  • Ability to travel to client homes as assigned.
