About the Role:
We are seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing direct patient care services to hospice patients and supportive care to their families. You will also act as a primary liaison between physicians, patients, and the hospice team, ensuring seamless coordination of care.

Responsibilities:
* Provide direct patient care services to hospice patients
* Coordinate patient care plans with the interdisciplinary team
* Act as primary liaison between physicians, patients, and the hospice team
* Empower patient caregivers through education and support
* Collaborate with an interdisciplinary group to provide comfort, peace, and dignity to patients

Requirements:
* RN license in the state of employment (or eligibility to obtain)
* 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
