Job Summary:
We are seeking an experienced Telehealth Nurse Specialist to join our team. As a Telehealth Nurse Specialist, you will provide high-quality patient care through telephonic triage and medical advice, working closely with our clinical team to ensure seamless care coordination.
Responsibilities:
- Provide patient telephonic triage and medical advice, adhering to established protocols and standing orders.
- Consult with providers on complex or difficult patients, ensuring timely and effective care.
- Schedule urgent visits as appropriate, collaborating with the clinical team to prioritize patient needs.
- Assess, interpret, and communicate lab results in coordination with medical providers.
- Review and refill prescription medication requests in accordance with established protocols.
- Respond to external provider inquiries and address them accordingly.
- Resolve and monitor patient problems and complaints, serving as a patient advocate for HIPAA and evidence-based practice.
- Perform clinical tasks requiring an RN or RN oversight, as needed.
- Administer and document patient history, symptoms, assessments, tests, and care according to guidelines.
- Ensure timely documentation of assessments and interventions, using appropriate CPT and ICD coding.
- Participate in daily huddles to proactively identify and meet patient needs.
- Administer and document immunizations in accordance with North Carolina Immunization Registry (NCIR) and Vaccines for Children (VFC) program(s).
Requirements:
- A minimum of 2 years' experience in an outpatient setting or public health center.
- Strong computer skills, including Microsoft Word, Excel, and Performance Management Systems.
- Bilingual English/Spanish skills are preferred.
- Excellent communication and data entry skills, with a strong focus on accuracy.
Compensation:
The estimated salary for this position is $65,000 - $80,000 per year, based on experience and location.
