Case Manager

Texoma Community Center seeks a skilled Case Manager to provide rehabilitation services to individuals with persistent and severe mental illness. The ideal candidate will possess a strong understanding of trauma-informed care and recovery-oriented models of service delivery.
Key Responsibilities:
Provide case management services to individuals with mental health needs
Develop and implement individualized service plans to promote recovery and well-being
Collaborate with community stakeholders to ensure seamless service delivery
Maintain accurate records and reports to meet program requirements
Requirements:
Bachelor's degree in Social Work, Sociology, Psychology, or Counseling
2+ years of experience in human services
Strong knowledge of trauma-informed care and recovery-oriented models
Excellent communication and interpersonal skills
Ability to work in a fast-paced environment
