LPN LVN Job Description

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

Key Responsibilities:
  • Provide whole-person care to patients, addressing their physical, emotional, and social needs.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies.
  • Administer medications, perform treatments, and provide wound care as needed.
  • Document patient progress, communicate with the healthcare team, and maintain accurate records.
Requirements:
  • Current California LVN licensure.
  • One year of experience as a Licensed Vocational Nurse.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift, push, and pull up to 25 pounds.
