We are seeking a skilled Physical Therapist Assistant to join our team at Tufts Medicine Care at Home. As a key member of our interdisciplinary team, you will work closely with licensed Physical Therapists to provide high-quality rehabilitative care to patients in various settings, including inpatient, transitional care, and outpatient settings.
Key Responsibilities- Provide patient care and services as planned, delegated, and supervised by the Physical Therapist, in accordance with the plan of care.
- Document services in compliance with agency policies and professional standards.
- Prepare visit notes for each visit, including therapy provided, patient response, teaching performed, and objective data.
- Communicate patient condition, response to services, and need for plan of care changes to the licensed therapist.
- Plan sessions within a reasonable time after receiving notification and at times beneficial to the patient.
- Complete required documents according to quality and industry standards in a timely manner.
- Report changes in patient condition and review with the registered therapist.
- Work effectively with patients, families, co-workers, and other community agencies/resources.
- Educate patients and families per the care plan.
- Participate in IDT meetings in care coordination.
- Comply with policies and procedures, including notification of schedule changes and accurate completion of time/day sheets.
