Job Title: Registered Nurse
Job Summary:
Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Registered Nurse to deliver care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.
Responsibilities:
- Provide treatment and healthcare procedures ordered by a physician and required by the standards of professional practice or law.
- Ensure individuals attend medical appointments per their Individual Support Plan (ISP).
- Advise supervisor of all concerns and issues related to the provision of nursing services.
- Create and distribute medical/health reports detailing general health, healthcare goals, and physician recommendations.
- Conduct programmatic trainings including medication administration and delegation.
- Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments.
- Train employees on the medical needs of assigned individuals.
- Monitor health and medical conditions of individuals and refer for medical services as needed.
- Monitor the use of medications through medication administration logs.
- Ensure health service supplies are available and replenished as needed.
- Ensure program documentation and billable records are completed accurately and timely.
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
- Participate with individuals in community, social, and recreational activities.
- Participate in circles of support for personal futures planning.
- Follow strict OSHA procedures regarding blood-borne pathogens.
- Employees must cooperate with the licensee and department staff in any inspection, inquiry, or investigation.
- Additional tasks and responsibilities may be assigned.
Requirements:
- Bachelor's Degree or equivalent experience in the nursing field.
- Minimum of one year of experience providing nursing services to individuals with disabilities.
- Valid R.N. license.
- Valid driver's license with a satisfactory driving record.
- Complete all state and agency required training per state guidelines.
- Ability to calculate figures related to medication dosages.
- Understanding and commitment to community-based support for persons with disabilities.
- Ability to exercise good judgment and remain calm in crisis situations.
- Ability to establish a comfortable and supportive relationship with an individual receiving supports.
- Proficient with Microsoft Office.
- Excellent verbal and written communication skills.
- Ability to effectively present facts and recommendations.
- Ability to effectively communicate with program participants, third-party agencies, staff, and members of the community.
- Excellent time management skills.
Working Conditions:
- Work in office, residential programs, day programs, and community environments.
- May be exposed to changing weather conditions when working outside.
- Hours and weekly schedules may change due to business needs and may include evening and weekend hours.
- May be required to be on-call in cooperation with other management staff.
- Occasional overtime may be required.
- Frequent lifting, stretching, and other physical exertion may be required.
- May be required to lift or move 50+ pounds.
- May occasionally assist with wheelchair transfer of non-ambulatory individuals.
- May be required to transport individuals utilizing your own vehicle or company-provided vehicles.
- May be exposed to various medical conditions, communicable diseases, blood, body fluids, or tissues.
