Job Title: LPN Home Health Assistant

LHC Group is seeking a skilled LPN Home Health Assistant to join our team in Shreveport, Louisiana.

Job Summary

We are looking for a dedicated and compassionate LPN to assist our Patient Care Manager in coordinating a team of staff to provide high-quality care to home health patients. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Responsibilities
  • Assist the Patient Care Manager in coordinating a team of staff to provide high-quality care to home health patients.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders.
  • Assist in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.
Requirements
  • Current LPN licensure in the state of practice.
  • Current CPR certification.
  • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
