Position Overview:
We are seeking a dedicated Registered Nurse Care Manager to deliver exceptional services at our Community Based Outpatient Clinic. In this pivotal role, you will advocate for Veteran patients, coordinate care with various stakeholders, and provide essential clinical services. Your contribution is vital for ensuring high-quality care and seamless continuity of services.
Core Responsibilities:
- Administer nursing care following directives from physicians and clinic management
- Oversee support personnel and facilitate PACT team meetings
- Work closely with Primary Care Physicians to guarantee quality care and continuity between the clinic and the VA medical center
- Uphold infection control protocols and deliver prompt patient care
- Assist in appointment scheduling and educate patients and their families
- Engage in Performance Improvement initiatives and ensure precise documentation
- Adhere to all regulatory requirements and maintain patient confidentiality
- Execute additional responsibilities as needed
Qualifications:
- At least 3 years of experience in Medical Surgical Units, Outpatient Facilities, Primary Care, or Adult Health
- Valid and unrestricted nursing license
- Current CPR/BLS certification
- Proficient in assessing patient conditions, responding to emergencies, and providing education
- Strong knowledge of patient treatment, customer service, and effective communication skills
Education and Experience:
- Graduate from a NLN-accredited nursing program; BSN preferred
Work Environment:
- Moderate physical activity; office environment
- Required to respond to medical emergencies within the clinic
