Job Summary

We are seeking a skilled RN Infusion Liaison to provide direct nursing services related to infusion therapy for patients in various settings. As a key member of our team, you will be responsible for providing education to patients discharging from area hospitals, as well as providing infusion nursing care in the home or clinic.

Key Responsibilities
  • Provide direct nursing services related to infusion therapy for patients in hospitals, homes, clinics, or alternate living sites.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
  • Provide education to patients and caregivers on infusion therapy, medication management, and self-care techniques.
  • Conduct regular assessments to evaluate patient progress and adjust care plans as needed.
Requirements
  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License upon hire (or Washington Registered Nurse License or National Registered Nurse Compact License).
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Unencumbered Registered Nurse License in state(s) of hire, and wherever care is delivered.
Preferred Qualifications
  • 1 year of experience with acute hospital care, infusion, or home health.
