Job Summary

Key Responsibilities
  • Linkage Services: Provide comprehensive linkage services to clients, families, community partners, and referral sources, including resource identification, outreach, and connection to Centerstone services.
  • Emergency Mental Health Consultations: Offer emergency mental health consultations at office locations to resolve mental health crises and provide timely support to residents in our service area.
  • Follow-up Appointments: Conduct follow-up appointments with clients as clinically appropriate, providing brief therapy and facilitating referrals to ongoing treatment or support services.
  • Documentation and Compliance: Maintain accurate and timely documentation, adhere to contracts, grants, standards, regulations, and laws associated with this position, and work under the direction of the Assessment Clinical Coordinator.
  • Team Collaboration: Participate in cross-functional teams, performance improvement initiatives, and staff meetings to ensure effective communication and collaboration with colleagues.
Requirements
  • Education: Bachelor's Degree in a Human Services field or equivalent with MHP status.
  • Experience: Experience working with the public or a demonstrated interest in serving diverse populations.
  • Certifications: Valid driver's license, safe driving record, and CPR certification within 90 days of hire.
