Job Summary

We are looking for a dedicated and experienced Nurse Practitioner to provide direct patient care, conduct comprehensive health assessments, and manage complex disease states. The ideal candidate will have a strong foundation in clinical experience, a patient-centered approach, and excellent communication skills.

Responsibilities
  • Work collaboratively with physicians and other healthcare providers to provide comprehensive patient care
  • Perform hospital rounds and assist with discharge planning
  • Conduct comprehensive health assessments, including physical exams and non-invasive preventive screening procedures
  • Identify medical and health risks and update patient records accordingly
  • Formulate differential diagnoses and order diagnostic tests and laboratory studies as needed
  • Develop and implement patient education plans and prescribe appropriate pharmacologic and non-pharmacologic therapies
  • Make referrals to other healthcare professionals and community agencies as necessary
  • Participate in quality assurance review and continuous quality improvement activities
Requirements
  • Graduate of an accredited Nurse Practitioner Program
  • NYS licensed
  • 2-5 years of related experience or equivalent combination of education and experience
  • Ability to utilize advanced health assessment and decision-making skills
  • Excellent communication and interpersonal skills
