Job Summary

The Social Worker will be responsible for providing medically-related social services to our residents, with the goal of improving their physical, mental, and psychosocial well-being.

Key Responsibilities
  • Admissions and Social Services Evaluations: Complete thorough assessments of residents' social and emotional needs, and develop personalized care plans to address these needs.
  • Financial and Legal Support: Assist residents with financial and legal matters, such as applying for pensions, referrals to lawyers, and referrals to funeral homes for preplanning arrangements.
  • Discharge Planning: Coordinate with residents, families, and healthcare providers to ensure smooth transitions to other care settings, such as home care services or other facilities.
  • Counseling and Psychiatric Services: Provide or arrange for counseling services and psychiatric medication management as needed, to support residents' mental health and well-being.
  • Documentation and Record-Keeping: Maintain accurate and timely documentation of residents' social services, including care plans and progress notes.
Qualifications

To be successful in this role, you will need:

  • Master's degree in Social Work or related field.
  • Experience working in a healthcare setting, preferably in a social work or patient advocacy role.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality care and services.
