Key Responsibilities- Provide comprehensive primary care services to patients, including health promotion, disease prevention, and management of acute and chronic conditions.
- Conduct thorough physical exams, take medical histories, and order diagnostic tests as needed.
- Prescribe medications and develop treatment plans to meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Valid state driver's license and clean driving record.
- Active, unrestricted state license, DEA license, and board certification.
- Minimum 2 years of clinical experience in primary care, geriatrics, or internal medicine.
- Strong technical skills, including proficiency in electronic medical records (EMRs) and HIPAA-secure messaging apps.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Master's degree in nursing or a related field.
- Experience in house calls, urgent care, or primary care settings.
- Knowledge of value-based care and HEDIS quality metrics.
