About the Role:

Maximus is seeking a skilled nurse to provide high-quality care and assessments to vulnerable populations in the Bronx.

As a Field Nurse Assessor, you will have the opportunity to work with a clinical leadership team to conduct home assessments, using the UAS-NY assessment tool to determine eligibility for community-based long-term services.

With a strong focus on delivering exceptional patient care, you will establish positive relationships with consumers, caregivers, and facility staff, ensuring accurate assessments and effective care plans.

Key Responsibilities:

  • Travel to conduct home assessments, using a clustered field visit approach to minimize travel time and maximize efficiency.
  • Perform in-home, in-facility, or telehealth assessments, evaluating individual health status, strengths, care needs, and preferences using the UAS-NY assessment tool.
  • Establish and maintain positive relationships with consumers, caregivers, and facility staff, ensuring accurate assessments and effective care plans.
  • Regularly review medical documentation, reporting any concerns, conflicting information, or issues that may arise during the evaluation process.

Requirements:

  • Current Registered Nurse (RN) license valid in the state of practice is required
  • Bachelor's degree and 5 years of experience or equivalent combination of education and experience required
  • Minimum of 5 years of clinical experience
  • Minimum 1 year supervisory experience preferred

Annual Salary Range: $107,000 - $118,000
