Position Overview:

The Hospital Access Liaison (HAL) plays a crucial role in supporting individuals who have recently been released from an Emergency Department (ED) or inpatient care. This position is dedicated to providing immediate assistance in connecting these individuals with essential community services and resources. The primary focus is on those at elevated risk of hospitalization, re-hospitalization, or suicide, ensuring they receive the necessary support.

Employment Details:

This is a full-time position with benefits, operating Monday through Friday, from 8:00 am to 5:00 pm. This role offers a hybrid work environment.


Key Responsibilities:


• Deliver prompt, hybrid support and care for individuals transitioning from hospital settings.

• Co-locate at hospitals as required to facilitate care.

• Implement short-term interventions for up to 30 days following initial contact, ensuring a smooth transition to ongoing services.

• Maintain regular communication with hospital personnel to promote continuity of care, address referral processes, and resolve any challenges.

• Schedule client appointments within the Electronic Health Record (EHR) system.

• Monitor client attendance at appointments 7 and 30 days post-discharge.

• Participate in monthly meetings and weekly clinical supervision with the HAL Supervisor.

• Uphold agreed productivity levels and quality indicators.

• Provide culturally sensitive services tailored to the unique needs of each client.

• Stay informed about all services offered by Compass Health.

• Be aware of other human and social service agencies for referral purposes when Compass cannot provide specific services, and document such referrals.

• Maintain client records in compliance with agency and state regulations.

• Ensure all documentation is completed in a professional and proficient manner.

• Collaborate with various programs to ensure appropriate referrals and enrollment in services.

• Communicate effectively with the multi-disciplinary treatment team of referred clients, if applicable.

• Perform additional duties as assigned by the supervisor.

• Travel may be necessary for co-location, team meetings, or agency-wide events.
