Position Description

The Substance Abuse Housing Liaison serves as a critical link between individuals struggling with addiction and the resources they need to achieve stable housing. This role requires exceptional relationship-building skills, a deep understanding of substance use disorder treatment options, and a passion for empowering individuals to take control of their recovery.

Main Responsibilities:

  • Connect individuals struggling with addiction to local housing resources, including emergency shelters, transitional housing, and permanent supportive housing.
  • Collaborate with treatment centers, peer support groups, and other organizations to ensure seamless transitions from treatment to housing.
  • Provide education and support on housing stabilization techniques, relapse prevention, and healthy coping mechanisms.
  • Maintain accurate records of client interactions, service delivery, and outcome tracking.
  • Participate in regular case conferences and team meetings to share knowledge and best practices.

Desirable Qualifications:

  • Minimum 2 years of experience working in substance abuse treatment or related fields.
  • Knowledge of Medicaid, NYCHA, and other relevant benefit systems.
  • Strong advocacy and crisis management skills.
  • Proficiency in Word, Excel, and database management software.
