About the Role

We are looking for a dedicated Medical Director who has experience in managing patients with multiple chronic diseases. As a key member of our team, you will be responsible for providing high-quality medical care, collaborating with our multidisciplinary team, and driving excellent patient outcomes.

  • This is a full-time position, working Monday through Friday from 8am to 5pm
  • You will see approximately 8-10 patients per day, with a focus on complex care management
  • The phone call will be shared among team members, with a rotation of 1 in 5 calls
  • You will work closely with a nurse practitioner, medical assistant, nurse care manager, clinical social worker, and office coordinator to deliver comprehensive care
