Job Title: Physical Therapist (PT) - Home Health

We are seeking a highly skilled Physical Therapist (PT) to join our team in providing in-home physical therapy services to patients recovering from illness, surgery, or injury.

Job Responsibilities:
  • Conduct comprehensive assessments of patients' physical abilities, mobility, strength, balance, and functional limitations to develop personalized treatment plans.
  • Design and implement therapeutic interventions, including exercises, gait training, and mobility techniques, to improve patient strength, coordination, and range of motion.
  • Educate patients and their families on home exercise programs, techniques for improving mobility, and safety measures to prevent falls or injuries in the home.
  • Collaborate with physicians, nurses, and other members of the healthcare team to ensure a coordinated and comprehensive approach to patient care.
  • Regularly assess and monitor patient progress, adjusting treatment plans as needed to ensure optimal recovery and achievement of therapy goals.
  • Document patient evaluations, treatment plans, and progress notes in the electronic health record (EHR), ensuring compliance with home health regulations and standards.
  • Provide recommendations for adaptive equipment, such as walkers or grab bars, to support patient mobility and safety at home.
  • Participate in discharge planning, ensuring patients receive continued support or outpatient therapy services as needed after home health care.
  • Stay up-to-date with advancements in physical therapy, home health best practices, and rehabilitation through continuing education and professional development.
Requirements:
  • Strong understanding of physical therapy principles, rehabilitation techniques, and home health care protocols.
  • Excellent communication and interpersonal skills for working with patients, families, and healthcare teams in a home health environment.
  • Proficiency in conducting assessments, creating individualized therapy plans, and implementing physical therapy interventions.
  • Active [STATE] Physical Therapist (PT) License in good standing.
  • A minimum of 2 years of experience as a Physical Therapist (PT), preferably in a home health or rehabilitation setting.
  • Completion of an accredited physical therapy program (Master's or Doctoral degree).
  • Certification in Basic Life Support (BLS).
  • Experience with home health documentation systems (e.g., EPIC, Homecare Homebase, etc.).
