About the Role

As a Pharmacy Navigator at Hackensack Meridian Health, you will play a critical role in ensuring our patients receive the medications they need to stay healthy. This is a dynamic and rewarding position that requires strong communication and problem-solving skills.

Key Responsibilities
  • Obtain patient authorization to process discharge prescriptions through the ambulatory pharmacy
  • Engage with floor nurses, discharge planners, case managers, social workers, physicians, and other teams to ensure patients receive critical medications in a timely manner
  • Determine information sources necessary to process prescriptions, including insurance coverage
  • Identify and troubleshoot discharge patient medication access issues related to insurance, financial, or insurance coverage
  • Adhere to all established HMH Pharmacy and Federal/State privacy and compliance policies to protect patient confidentiality
  • Ensure prescription fulfillment by in-house pharmacy based on priority needs to allow for timely discharge
  • Coordinate and facilitate patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas
  • Ensure patients receive an offer for pharmacist counseling and distribute pharmacist contact information in the event of prescription-related questions at home
  • Facilitate distribution of medication-related educational materials and coordinate counseling with staff pharmacist as needed
  • Assist Ambulatory Pharmacy staff as needed
  • Perform duties of Ambulatory Pharmacy Technician if required
  • Ensure all guests/patients receive safe care and maintain a safety-first work ethic
Requirements
  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs
  • Excellent written and verbal communication skills
  • Proficient computer skills, including Microsoft Office and/or Google Suite platforms
  • Pharmacy Technician License
  • PTCB or ExCPT Certification Required within 180 days of start of employment
