About the PositionWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve health outcomes.
Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care.
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
- Masters Degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certified by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
Ability to travel, valid state driver's license, and ability to drive a car with proof of adequate automobile insurance coverage for the state of residence. Travel required to meet patients where they live.
