Job Title: Registered Nurse (RN)

Alto Health Care Staffing is seeking a skilled Registered Nurse (RN) to join our team in a skilled nursing facility and rehabilitation center setting.

Responsibilities:
  • Patient Care:
    • Conduct comprehensive nursing assessments and develop individualized care plans.
    • Administer medications and treatments as ordered by physicians.
    • Monitor patient conditions and initiate appropriate interventions.
    • Provide wound care, ostomy care, and other specialized treatments as needed.
    • Educate patients and families about disease processes, medications, and self-care.
    • Collaborate with the rehabilitation team to develop and implement patient rehabilitation plans.
  • Leadership and Supervision:
    • Supervise LPNs and CNAs in providing patient care.
    • Delegate tasks appropriately and ensure staff compliance with facility policies and procedures.
    • Participate in the hiring, training, and evaluation of nursing staff.
  • Quality Improvement:
    • Participate in quality improvement initiatives to enhance patient care outcomes.
    • Identify and report potential or actual incidents and adverse events.
    • Collect and analyze data to identify trends and opportunities for improvement.
  • Documentation:
    • Accurately and timely document patient assessments, interventions, and outcomes in the electronic health record.
    • Complete required documentation, including incident reports, medication administration records, and progress notes.
  • Collaboration:
    • Collaborate with physicians, therapists, social workers, and other healthcare professionals to provide coordinated patient care.
    • Participate in interdisciplinary care planning meetings.
  • Compliance:
    • Ensure compliance with state and federal regulations, including OSHA, HIPAA, and CMS guidelines.
    • Maintain current licensure and required certifications.
Qualifications:
  • Graduate of an accredited nursing program.
  • Current and valid Registered Nurse (RN) licensure in the state of Ohio.
  • Experience in long-term care, skilled nursing, and rehabilitation preferred.
  • Strong clinical assessment and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a healthcare team.
  • Demonstrated leadership and supervisory abilities.
  • Proficient in electronic health record documentation.
