Alto Health Care Staffing is seeking a skilled Registered Nurse (RN) to join our team in a skilled nursing facility and rehabilitation center setting.
Responsibilities:- Patient Care:
- Conduct comprehensive nursing assessments and develop individualized care plans.
- Administer medications and treatments as ordered by physicians.
- Monitor patient conditions and initiate appropriate interventions.
- Provide wound care, ostomy care, and other specialized treatments as needed.
- Educate patients and families about disease processes, medications, and self-care.
- Collaborate with the rehabilitation team to develop and implement patient rehabilitation plans.
- Leadership and Supervision:
- Supervise LPNs and CNAs in providing patient care.
- Delegate tasks appropriately and ensure staff compliance with facility policies and procedures.
- Participate in the hiring, training, and evaluation of nursing staff.
- Quality Improvement:
- Participate in quality improvement initiatives to enhance patient care outcomes.
- Identify and report potential or actual incidents and adverse events.
- Collect and analyze data to identify trends and opportunities for improvement.
- Documentation:
- Accurately and timely document patient assessments, interventions, and outcomes in the electronic health record.
- Complete required documentation, including incident reports, medication administration records, and progress notes.
- Collaboration:
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to provide coordinated patient care.
- Participate in interdisciplinary care planning meetings.
- Compliance:
- Ensure compliance with state and federal regulations, including OSHA, HIPAA, and CMS guidelines.
- Maintain current licensure and required certifications.
- Graduate of an accredited nursing program.
- Current and valid Registered Nurse (RN) licensure in the state of Ohio.
- Experience in long-term care, skilled nursing, and rehabilitation preferred.
- Strong clinical assessment and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a healthcare team.
- Demonstrated leadership and supervisory abilities.
- Proficient in electronic health record documentation.
