Nurse Care Manager

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Family Health Centers of San Diego. As a Nurse Care Manager, you will play a vital role in providing exceptional healthcare to our patients, particularly those in underserved communities.
Key Responsibilities- Conduct initial assessments for potential PACE participants, including home visits to support environmental and physical assessments.
- Ensure coordination and completion of reassessments every 6 months with 1:1 at PACE Multipurpose Center and/or at participant's home for assigned case load.
- Act as a liaison between participants and the interdisciplinary team members.
- Provide individual health education on the disease process, including verbal and written hand-outs for participants' reference.
- Document any activities encountered between participants, families, caregivers, contracted providers, hospitals, SNF, and other facilities working together with PACE for participants' well-being in accordance with PACE policies, procedures, and all regulatory requirements and laws.
- Ensure implementation and compliance of participant care as per provider's order.
- Respond to medical emergencies and take the lead in management of the situation as indicated.
- Screen and triage participants as needed.
- Document all nursing care on the day service is rendered while adhering to established guidelines related to medical care provision.
- Monitor, administer, and record ordered medications.
- Participate as an active member of the interdisciplinary team to assess the nursing need of the participants.
- Maintain acceptable standards of nursing care based on regulatory requirements.
- Perform other duties as assigned.
- Current California state licensure as a Registered Nurse.
- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
- Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
- 1-3 years of experience working with the older adult population required.
- 1-3 years of experience in managed care setting to include utilization or case management in an ambulatory or inpatient setting preferred.
- Knowledge of the older adult population health care issues.
- Knowledge of and ability to perform clinic-based/or hospital-based/or home-based nursing tasks.
- Ability to work effectively in a team environment.
- Ability to participate collaboratively with multi-disciplinary care team.
- Ability to work effectively with participants from diverse social, cultural and economic groups.
- Must be able to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high workload environment.
- Basic computer literacy to comply with department needs and expectations (i.e., E-HR documentation, obtaining background information and reports, following up on appointments, etc.).
- Bilingual English/Spanish preferred.

$49.95
If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience.
Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
