Chronic Care Case Manager RN

As a Chronic Care Case Manager RN at Grand River Health, you will play a vital role in enhancing efficient care for our patients. Working under the direction of clinic leadership, you will collaborate with the patient's provider and team to emphasize the patient's central role in managing their health. Your proactive approach will involve regular, planned visits and phone calls to help individuals maintain health and provide resources for optimal chronic disease management.
Responsibilities:
- Schedule appointments and follow-ups as the patient's condition requires, providing patient education regarding their medical condition.
- Screen and respond to patient phone calls, directing to primary care provider as needed.
- Furnish medical advice per collaborating providers, scheduling appointments based on necessity.
- Schedule urgent visits as appropriate.
- Enter necessary information into the EHR and accurately document the encounter with the patient.
- Review health records of identified chronic disease patients and adhere to disease-specific treatment guidelines and objectives.
- Order appropriate testing/referrals as needed and send the information to the PCP for verification.
- Track completion of testing/referrals and organize data.
- Implement process improvement or quality improvement initiatives to maximize outcomes.
- Maintain confidentiality of all patient information and adhere to clinic policies and safety practices.
- RN license in the state of Colorado.
- Current BLS card within 3 months of initial employment and maintain current status throughout GRHD employment.
- Minimum of five years of experience as an RN working with primary care or internal medicine chronic disease management.
